SFS FELLOWS NOMINATION FORM

Fellows Nomination Form

SFS Fellows nominations submitted by November 15 of each year will be considered for the Fellows class of the subsequent calendar year. Nominations will be considered for one year only, but the same person may be re-nominated again in future years.

Nominations should include (1) a nominating letter and (2) one or two letters of support compiled into a single document.
